Form I-693 Immigration Exam / Green Card Medical Exams / Adjustment of Status / USCIS Medical Exam
Family Urgent Care provides immigration physicals for the entire family. Dr. Arriviello is our Civil Surgeon who provides these exams 7 days a week. We can get you in the same day or the next day. Our very competitive price includes the mandatory Quantiferon Gold Tuberculosis (TB) test (for 2 years old and older), syphilis blood test (18-44 years old), and gonorrhea urine test (18-24 years old). If your immunizations are not up to date or you do not have a record of such, we can do additional blood tests to determine your immunity for an additional cost. We will match any reasonable price once verified.
Fees:
Our one low price of $325 for adults 18 years and over and $250 for those under 18 years old, includes the initial and follow-up visits as well as flu vaccination during flu season (10/1 – 3/31).
If the Tuberculosis test is positive, then we would need to do a mandatory chest x-ray to prove to USCIS that you do not have active tuberculosis. That is an additional charge of $75 but only if the TB test is positive.
Vaccine Requirement for Adults 18 years old and older:
- Tetanus (Td or Tdap) within the past 10 years
- IPV Polio vaccine – 4 doses
- MMR (measles, mumps, rubella) – 2 doses
- Varicella (chicken pox) – 2 doses or we can also document “Varicella History” if you had the chicken pox.
- Hepatitis B – 3 doses (up to age 59)
- COVID vaccine – 2 doses of Pfizer or Moderna or 1 dose of J&J / Janssen or 1 dose of current vaccine.
- Influenza (flu) vaccine only during flu season which is September 1 – March 31
- *If 65 years old or older you must have 1 dose of pneumococcal vaccine (Prevnar-20)
Vaccine Requirement for children 2 months – 17 years old:
- Need the above based on age and:
- Hepatitis A vaccine – 2 doses (12 months – 18 years old).
- Meningococcal serogroup A, C, W, Y vaccination – (minimum age: 2 months [MenACWY-CRM, Menveo], 2 years [MenACWY-TT, MenQuadfi]), 10 years [MenACWY-TT/MenB-FHbp, Penbraya]) 2 doses 11-12 years old; 16-18 years old.
- Pneumococcal vaccine (PCV 15, PCV 20) – 4 doses from 2 – 18 months (only 1 dose is needed if given after 24 months old)
- Hib vaccine – 4 doses from 2-59 months old
- Rotovirus – 2 doses from 2-4 months old
- COVID – from 6 months old and older
As of 10/01/21 proof of COVID vaccination is required for everyone 6 months of age and older.

Are vaccines mandatory?
Yes. You may bring vaccine records in with you or email to info@familyurgent.care and Dr. Arriviello will review to see if your records are complete. We may need to do additional lab tests to check for immunity.
What if I refuse to get the required vaccines?
You may apply for a waiver if your objection is based on religious beliefs or moral convictions. You would need to complete form I-601 (https://www.uscis.gov/sites/default/files/document/forms/i-601.pdf)
There is no guarantee, and your adjustment of status may be denied.
What do I need to bring to my appointment?
Photo ID (driver’s license, passport, Gov’t issued ID)
Copy of vaccine records, old I-693 form, any letters from USCIS
How long does the process take?
It is a minimum of 2 visits. The first visit is approximately 45-60 minutes and involves filling out I-693 form, obtaining blood and/or urine tests and giving any vaccines. Once all the lab results are obtained (2-4 business days) then the process can be completed by signing the I-693 (parents or guardians can sign for those under 14 years old). Obtain any additional vaccines that may be needed. A copy of the signed I-693 will be given to you along with any lab results before the original is sealed in an envelope. The sealed envelope is what gets sent to USCIS by yourself or attorney.
